From Secret to Spotlight: How Pidgeon Pagonis Is Redefining Intersex Power in the LGBTQIA+ Community

If the early pioneers of the intersex movement were tasked with breaking the silence, the modern generation is tasked with tearing down the institutions that enforce it. At the forefront of this shift is Pidgeon Pagonis, a Chicago-based activist, filmmaker, and author whose fierce organizing has permanently altered the LGBTQIA+ landscape, helping move intersex advocacy into a central pillar of modern queer liberation.

Surviving the "Medical Industrial Complex"

Like many intersex people born in the late 20th century, Pagonis’s early life was dictated by deception. Assigned female at birth, they underwent a series of invasive medical procedures. Doctors told Pagonis a fabricated story: that they had ovarian cancer and needed surgeries to stay healthy. In reality, Pagonis had internal testes and a natural intersex variation.

The truth didn't emerge until Pagonis was in a college gender studies class. When the professor introduced androgen insensitivity syndrome, the pieces violently clicked into place. Pagonis realized their childhood surgeries weren't life-saving cancer treatments—they were cosmetic, non-consensual procedures designed to forcefully fit their body into a rigid male/female binary.

Taking Back the Narrative: Art as Activism

Pagonis refused to let their story stay hidden, later turning the medical community's mandate of secrecy into the title of their acclaimed 2023 memoir, Nobody Needs to Know. By utilizing film, photography, and digital media, they gave the movement a highly visible face:


  • The Son I Never Had (2014): A breakthrough documentary weaving personal storytelling with medical records to expose the trauma inflicted on intersex youth.

  • National Geographic (2017): Pagonis appeared on the historic "Gender Revolution" cover, bringing intersex visibility to millions worldwide.

  • Digital Campaigns: They launched the first global Twitter/X storm on Intersex Awareness Day, using #intersexstories to create a digital community where isolated individuals could find instant solidarity.

The Intersex Justice Project: Demanding Accountability

Knowing that changing culture requires institutional reform, Pagonis co-founded the Intersex Justice Project (IJP) with Sean Saifa Wall, driving the #EndIntersexSurgery mission. Adopting the direct-action tactics of broader queer and civil rights movements, they set their sights on Chicago’s Lurie Children’s Hospital with years of protests and public pressure campaigns.

The strategy worked. In 2020, Lurie Children’s Hospital made history by becoming the first major U.S. medical institution to publicly apologize and officially halt cosmetic, non-consensual surgeries on intersex infants.

Bridging the Gap in the LGBT Movement

Pagonis’s work has been vital in centering the "I" in LGBTQIA+, which was historically treated as an afterthought. Identifying as queer and non-binary, Pagonis fiercely advocates for the intersections of trans and intersex liberation.

They frequently expose a glaring legislative hypocrisy in today's political climate: the politicians attempting to ban gender-affirming care for trans youth are often the exact same ones passing laws to protect non-consensual surgeries on intersex babies.

By framing both struggles as a unified fight for bodily autonomy, Pagonis proves that whether fighting for the right to transition or the right not to be forcefully transitioned at birth, the goal is the same. Named an LGBT Champion of Change by the White House, Pagonis continues to remind the queer community that true liberation means dismantling the binary systems used to police all of our bodies.
